Note:Children's passports can
not be renewed. Regardless of previous issuance, all children must
go through the new passport application process.
Process Overview
Applicant and both parents or legal guardian(s) must gather documents
listed below and make a personal appearance in front of an Acceptance
Agent.
An Acceptance Agent is a government employee who will verify
the identity of the applicant and parents or legal guardian(s).
Acceptance agents are at designated Post Offices and Court
Houses. Click

Applicant must indicate that he/she is using an expediting
service to "hand carry" the passport application
to the passport agency for processing. The Acceptance Agent will
have the applicant and parent sign the application and seal all
documents in an envelope and return the sealed envelope to parent
or legal guardian.
Sealed envelope is forwarded to BPS for processing.
Note: If only one parent or legal guardian
is available to appear in person, the government offers form DS-3053
which once executed allows for the parent or legal guardian to not
appear in person. The consent statement must include the child's
detailed information, the parent or legal guardians identification
information (photo copies of identification are highly encouraged)
and must be notarized. Only original copies are accepted. This requirement

Dropping off Your Documents in Person?
BPS is located near an Acceptance Facility open
from 9:00am to 4:00pm Monday through Friday. If you plan to drop
documents at our office in person, we recommend visiting our office
prior to seeing an Acceptance Agent so that we may review your documents.
Passport photos are also available in our office for your convenience.
Step 1 - Documents to be provided to
the Acceptance Agent
1.1
Proof of Citizenship
Applicant must bring an original or certified
copy of a birth certificate, naturalization papers or old
passport.
1.2
Parent or Legal
Guardian's Proof of Identity
Parent or guardian will be required to show
a driver's license or state-issued identification issued more
than six months ago. Official government or military picture
identification, valid U.S. or foreign passport with photo,
or an I.N.S. Alien Resident Card is also acceptable.
1.3
Proof of Relation
Parent or guardian must prove their relation
to the applicant. Generally this is done by matching the name
on a driver's license or state-issued identification to the
original birth certificate.
Other acceptable documents are Certified Foreign Birth Certificate
of the applicant (translated, if necessary); Report of Birth
Abroad of the Applicant: Form FS-240; Certification of Birth
Abroad of the Applicant: Form DS-1350; Adoption Decree of
the Applicant (with adopting parents' names); Court Order
Establishing Custody of the Applicant; Court Order Establishing
Guardianship of the Applicant; or similar legal documents.
NOTE: If the name of the parent/guardian
is other than that on the document, evidence of legal name
change is required.
1.4
Passport Photos
Applicant must submit two (2) identical passport
photographs. These may be obtained at many post offices, Kinkos,
CVS or similar locations.
NOTE: If applicant is delivering
documents to our office personally, passport photos can be
taken on-site.
1.5
Flight Itinerary
Applicant must submit a computer-generated
itinerary from a travel agency or online booking agency showing
departure from the U.S.
1.6
Authorization
Letter
Applicant must submit a "Letter of Authorization"
which allows BPS to submit the application
to the US State Dept.
Parent or legal guardian must pay government
fees at acceptance agent via check, cash, money order or debit
card (credit cards are not accepted). There will be two fees,
one payable to the U.S. Department of State for $125 and the
second for $25 payable to the acceptance agent facility.
1.8
Passport Application
Applicant must submit one completed new passport
application form (DS-11). Application must be filled out online
via US State Dept web site.
DO NOT sign the application until requested
to do so by the Acceptance Agent.
Send sealed envelope from Acceptance Agent
and documents listed in this section for processing.
