Requirements and Instructions for Name Changes
on Your Passport
In order to change the name on a US passport, applicant
must meet the following qualifications:
Applicant has US passport is in his or her possession;
And applicant has legally changed their name;
And applicant's passport was issued less than 15 years ago;
And applicant was 16 years or older when passport was issued.
NOTE: Modifying the name on a US passport
due to marriage, divorce or other legal purpose results in an early
passport renewal. A new passport will be issued with new passport
number. All current visas will be cancelled.

BPS offers photos on-site for your convenience.

Current Passport
Applicant must submit their current passport.
1.2
Name Change
Documentation
Applicant must submit original or certified
copy of the marriage certificate (not a license) or a court
decree ordering a name change or an adoption. The document
must show the official seal of the issuing authority. All
official documents will be returned.
1.3
Passport Photos
Applicant must submit two (2) identical passport
photographs. These may be obtained at many post offices, Kinkos,
CVS or similar locations.
NOTE: If applicant is delivering
documents to our office personally, passport photos can be
taken on-site.
1.4
Flight Itinerary
Applicant must submit a computer-generated
itinerary from a travel agency or online booking agency showing
departure from the U.S.

Authorization
Letter
Applicant must submit a "Letter of Authorization"
which allows BPS to submit the application
to the US State Dept.
The US State Dept charges $60 for expedited
visa pages.
This fee may be made by personal check made payable to the
US State Dept. or may be combined with your service fee payment
to BPS.

Passport Application
Applicant must submit one completed passport
application form (DS-4085). Application must be filled out
online via US State Dept web site then printed from the web
site once completed.
BPS Passports recommends Federal Express or UPS for document shipping.
